Participant Waiver & Release

GAMING TO YOU PARTICIPANT WAIVER & RELEASE

Please read before signing. This form applies to Gaming To You activities and equipment.

ASSUMPTION OF RISK
I understand that participation in Gaming To You activities involves inherent risks, including possible injury arising from physical activity, interaction with other participants, use of equipment, and the event environment. I knowingly and voluntarily assume the risks associated with participation, both known and unknown, and accept responsibility for my own safety and, when applicable, the safety of the minor participant(s) listed below.

RELEASE AND HOLD HARMLESS
To the fullest extent permitted by law, I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, release and hold harmless Gaming To You, its owners, officers, employees, agents and representatives from claims or liability for injury, disability, death, loss or damage to personal property arising from participation in Gaming To You activities, except to the extent such release is prohibited by applicable law.

ACTIVITIES, SAFETY RULES AND PARTICIPATION
I give permission for the participant(s) listed below to take part in Gaming To You activities, which may include the Gaming Trailer, Laser Tag, Nerf Warz, Knockerball, Light Saber activities, Yard Games, Movie Nights and other offered entertainment. I understand that activities may involve running, physical contact, moving equipment, electronic equipment or other activity-specific risks. I agree that all participants will follow instructions and safety rules provided by Gaming To You staff. A participant who does not follow safety rules may be removed from an activity. A participant may decline to participate in any activity at any time.

EQUIPMENT RESPONSIBILITY
Gaming To You may provide controllers, laser taggers, Nerf equipment, light sabers, Knockerballs, yard games or other equipment. I agree that the participant(s) will use equipment as instructed and return it in substantially the same condition as received, ordinary wear excepted. I accept responsibility for equipment that is intentionally damaged, misused, lost or not returned while assigned to me or the participant(s) in my care.

VIDEO / GAME CONTENT
When video gaming is part of the event, available titles may carry ratings from E (Everyone) through M (Mature). I understand that some game content may not be suitable for all ages. A parent or guardian may ask that a minor not participate in a particular game or may request age-appropriate alternatives when available.

ACKNOWLEDGMENT
I certify that I am the adult participant or the parent/legal guardian authorized to sign for the minor participant(s) identified in this waiver. I have read this waiver, understand its terms, and agree voluntarily.
